TEAMS MUST REGISTER AT LEAST TEN DAYS PRIOR TO AN EVENT.    ALL ENTRY FEES MUST BE RECEIVED AT OUR OFFICE BEFORE THE EVENT, OR THERE IS NO GUARANTEE THAT YOUR TEAM WILL EVEN BE GIVEN A SPOT.   A PENALTY / LATE FEE WILL BE ASSESSED AND ADDED TO ANY ENTRY FEES THAT ARE PAID AT THE EVENT.

ONCE YOU SUBMIT YOUR TEAM REGISTRATION, YOU WILL BE EMAILED A CONFIRMATION NUMBER.   IF YOU DO NOT RECEIVE A CONFORMATION NUMBER WITHIN THREE DAYS, TRY AGAIN OR CALL US AT 573-468-5347 EXT. 400.

Use this form for the Friday Night "63.5 PPD" Triples and the
Saturday Morning Handicapped 4 person "84.5 PPD" team registrations.

$10.00 Per Player currently Playing League,
$20.00 per player not playing, but has a history PPD.

HISTORY PPD FILE

For the Triples Event, you may sign up any three people who's current history PPD does not exceed 63.5.

For the
Handicapped Team Event, ou may sign up any four people who's current history PPD does not exceed 84.5.

Play-Mor Coin-Op and the Promotional Services League reserves the right to share any and all discussions, comments, complaints, input (written or verbal) as we understand the comment to mean or imply. We will use this policy at our discretion for the overall benefit of the Play-Mor Dart League program.
